I agree, I didn't think Sam was going the same place as Dean. I'd kind of pictured it as limbo or purgatory. I don't think Sam is/was being tortured except by the way Lucifer was--sort of the ultimate solitary confinement.
Ack! I don't want Sam to be a reaper!
